In designing effective mobile data collection forms, that allow for capturing GIS coordinates, here are a few simple rules to follow

Divide form into sections
If a data collection form is going to end up being lengthy, consider separating it into sections. A well-structured multi-section form keeps data fields better organized, and provides a helpful overview for data collectors.

Divide sections into sub-sections
Oftentimes for very lengthy data collection forms, there may be a need to further break up sections into sub-sections in order to achieve a well organized structure. The subsections are only for organizing the form, they typically share the same level and have equal importance.

Use skip logic
Skip logic, sometimes referred to as ‘branching’ or ‘relevant conditions’ will omit questions that are not relevant when a certain condition or conditions are met. By default, all questions are always visible. Skip logic controls which question should be displayed only if a certain condition (or conditions) is fulfilled. Conditions are always applied to the question or group that should be sometimes hidden, sometimes visible. (This is important, as many paper forms attempt to integrate a variation of skip logic by writing things like “If yes, go to question 22”.)

Well-designed skip logic means that data collectors never have to worry if the next item they see is relevant, saving time and avoiding confusion. The use of skip logic makes forms more logical and improves data quality.

Include fields to capture Lat/Long geographic coordinates
For performance indicators that have a spatial orientation, that is, they possess discrete geographic coordinates, locations or facilities where the activities and outcomes can be measured, mobile data collection forms should include fields to capture geographic coordinates. Capturing geographic coordinates requires no special equipment, today, most smart phones can accomplish this.
